To respect each patient's right to the confidentiality and privacy of their health care information, Aspirus requires signed authorization from the patient or legal guardian. The health care record is the property of Aspirus and is maintained for the use of our Hospitals, Clinics, Medical Staff and for the benefit of the patient. Use the information below to request your medical records.

Medical Record Charges

Fees for paper copies of medical records in Wisconsin and Michigan may apply and are subject to change in accordance with state law.

Actual postage costs will apply.

• To request records please submit your request through the Aspirus Requester Portal. 

• If you are common requester for treatment purposes only, you can sign up for EPIC Link.

This applies to groups that are not considered a health care provider, for example (Attorneys, Insurance companies, Human Service Departments, etc)
