How do I get documentation for my employer to return to work?

If you have already received your COVID-19 test results and need to provide documentation to your employer to return to work, DO NOT call the Aspirus COVID-19 Call Center. Our Call Center Staff cannot help you with the required documentation. Instead, the quickest way to get this documentation is to log on to your MyAspirus account from a computer. Find the "print icon" on the right side of the screen and print your results to a paper printer or an electronic PDF printer. Please note that this is not available on a cell phone or tablet. Screenshots do not show your name and date of birth and often are not accepted by your employer and/or school.

If you do not have a computer with a printer you will need to obtain a HIPAA Authorization form complete it and email it to aspirushealthinformation@aspirus.org to have health information share your results. This can take 2-3 days from the date that your HIPAA authorization has been received.
