What is an Advance Directive?
An Advance Directive is a document in which a person states goals, values, and beliefs about health care treatment decisions, including who should make those decisions, in the event that person can no longer make decisions for him/herself. Advance directives are for everyone age 18 and older.

Did You Know?
Wisconsin and Michigan are NOT next-of-kin states? This means family members are not automatically authorized to make health care decisions for you unless you have completed an Advance Directive (Power of Attorney for Health Care) naming them as the agent. Did you also know that if you don’t have an Advance Directive (Power of Attorney for Health Care) and you’re not able to make health care decisions for yourself, the court would likely have to get involved? This could cost your estate thousands of dollars.
