Community Health Needs Assessment

Every three years, Aspirus Health aligns resources with local area health systems and Health Departments to participate in a shared data collection process called the Community Health Needs Assessment (CHNA). This CHNAs serve as the foundation for each hospital’s implementation strategies to improve health outcomes and reduce disparities.

Community Health and Health Equity

Our three year focus from CHNA process includes addressing health equity through the lens of mental health and substance use. Outlined below are our shared initiatives across Aspirus communities:

    Helping people navigate the resources that they need, especially in the areas of social determinants of health. Search and connect for support.

  • Expanding “Raise your Voice Clubs” 

    Partnering with schools to recognize signs of mental health. Establishing peer advocate structures for students in and out of the classroom.



  • Offering Recovery Coaching

    Connecting with individual when they present in an Aspirus hospital, a warm handoff for navigating community recovery resources.
