Working collaboratively with your physician, we will design a comprehensive plan of care that is specific to your needs.

Your care will begin with an assessment to discuss the situation, decide what level of care you need and determine what outside sources – from your family to agencies in the community – are available to you.

Aspirus VNA Home Health Team Includes:

Skilled Nurses
A full range of nursing services including wound care, IV therapy, diabetic care, medication management, catheter care, health assessment, cardiac care and more.

Assessment and treatment by physical therapists, occupational therapists and speech therapists.

Medical Social Workers
Coordination with community support systems, assistance with advanced directives, living arrangement assessments and financial assistance information.

Home Health Aides
Assistance with daily living activities and personal care.

Rehabilitation Services

Physical Therapy
Our physical therapists assess and provide treatment to help patients of all ages increase mobility, strength, balance and coordination. They also work with family and community resources to maximize the rehabilitation process.

Occupational Therapy
Occupational therapists help patients improve their daily living skills such as eating, bathing and dressing. They may also train patients to use adaptive devices.

Speech Therapy
A speech therapist helps patients restore or improve their communication skills including reading, writing and speaking. They may also teach patients and family members safe and appropriate swallowing techniques.

Specialized Programs

Transition to Independence™
Will you need surgery in the near future? If so, you will want to prepare by including Transition to Independence™ in your plans. Post-surgery care is often more than one caregiver can provide alone. Weakness after surgery and post-effects of anesthesia in addition to medication and day-to-day activities sometimes requires additional assistance when you return to your home. Nurses, therapists or home health aides will come to assist you with things like medication management, exercise programs, and physical assistance.

Aspirus VNA Home Health is one of just a few agencies in Wisconsin to use this latest technology in home health care. Our Telemonitoring Program is an innovative service for patients at risk for hospitalization, newly diagnosed with a chronic disease or those who would benefit from daily evaluation by a licensed nurse.

The goals of Telemonitoring are:

  • Catch problems early before they turn into emergency needs
  • Decrease the need for hospital admissions
  • Help people manage their own health care at home
  • Provide peace of mind that a professional nurse is providing daily monitoring

Each day, the Telemonitoring unit placed in patients’ homes guides them through a simple, three-minute process with step-by-step instructions. People are able to record their weight, blood pressure, pulse, temperature, oxygen saturation, and blood glucose. The data is then securely transmitted to the Aspirus VNA office where a nurse reviews the information and tracks for significant change.

Based upon criteria established by the physician in advance, the nurse responds with telephone intervention as prescribed that includes further assessment, teaching or communication with the patient’s physician.


Aspirus VNA Home Health Services are covered by a variety of sources including: private pay, private insurance, Medicare, Medicaid, and other sources. Typically payment options cover specific services and have special criteria required prior to the availability of funds. A staff person from Aspirus VNA will work with you and your family to evaluate all payor options prior to receiving care.
