Appointments at Aspirus Westhill Medical Specialists
New patients will be asked to complete a form prior to their visit outlining their health history. This will help the providers make a prompt and accurate assessment during the first visit. Please return the form at least 3 days prior to your scheduled appointment.
“Wellness Exams” offer an opportunity for the patient to have a thorough evaluation of preventive health services pertinent for their age and risk. An emphasis on wellness and prevention as well as the development of a long range health wellness plan are part of this evaluation. Due to the comprehensiveness of this examination many times patients with multiple chronic or acute health issues may need those problems addressed at a separate, chronic condition focused appointment.
The goal of Aspirus Westhill Medical Specialists is to provide adults with prompt, easy access to quality health care. Our providers see patients by appointment during normal office hours. A triage nurse will discuss your medical needs with you and arrange for an appropriate appointment.
Chronic Condition Focused Appointments
Healthcare appointments and routine tests at regular intervals are an important part of good chronic condition care to protect against complications and other health problems.
These focused appointments allow your provider time to discuss all aspects of your
chronic condition with you. Because these conditions can require a lot of time to assess and discuss and because it is important to cover all aspects of your chronic condition, we ask that these appointment times are used for that intended purpose. If there are other health concerns that you wish to discuss, we ask you make separate appointment.
When you come to your appointment, please bring your current insurance cards (including Medicare cards) and any current prescriptions, supplements and over the counter medications. Please arrive 15 minutes before your scheduled appointment to complete any necessary registration information.
Urgent/Same-Day Medical Care
We understand that there are times when you will have a medical problem that needs urgent attention and you cannot wait for an appointment. If an urgent situation occurs during our office hours, call our office and discuss your needs with our triage nurse. Our triage nurse works closely with your provider and will assist you with your medical and appointment needs.
