Supervisory Referral

From time to time management or human resources need to address an employee’s work performance.  This employee has been an asset to you and has proven valuable to your company in the past.  Something has changed and this employee is showning decrease in the ability to effectively do his/her job.

The Supervisory Referral System from Aspirus EAS assists both management and employees by having the employee meet with a counselor in a neutral setting to discuss issues.  It is the counselor’s impartial perspective that can assist with identifying concerns as well as problem-solving strategies.

Informal Referral - The mildest referral option.
Management explains that work performance has decreased, is unacceptable, and needs to improve.  There is no follow-up between management and Aspirus EAS.

Formal Referral - The moderate referral option.
Management explains that work performance has decreased, is unacceptable, and needs to improve.  Management recommends the formal referral to Aspirus EAS to employee.  Aspirus EAS will inform management that an employee has scheduled and attended an initial appointment.  The employee has the option to continue to meet with the counselor to further discuss issues for up to a predetermined number of sessions.

Return to Work Referral - The strongest referral option.
Management explains that work performance has decreased, is unacceptable, and needs to improve.  Management recommends the return to work referral to utilize Aspirus EAS.  Aspirus EAS will inform management that employee has scheduled an appointment, and employee will continue to meet with counselor as long as the counselor deems necessary.  Management will receive a return to work agreement from Aspirus EAS.  Management and the employee will need to sign and date this contract.

To begin the Supervisory Referral process management will need a Supervisory Referral Form that sumarizes what management is asking for the employee to do.  This form should be completed prior to meeting with the employee.  Under the “Terms of Referral” section management will need to select which of the three options to use.
